For your Live Birth Registration,  This is the paperwork that needs to go to the Department of Foreign Affairs and Trade if you are having to send it off yourself.  My notary offers this step when documents need to go to DFAT.

    I’m looking for the correct Victorian Live Birth record request form the “Application for a Victorian original registration document’ resulted in an illegible second schedule being sent and cost $105.30! Where is the document I need please?
