✅ Type carefully — accuracy in wording, spelling, and format is essential.
✅ Complete every field exactly as requested — no more, no less.
✅ Review all details before submission; this becomes your official Commonwealth Record.
✅ Prepare and upload your red-ink signature image (clear, level, on white background).
Make the file name your Document Number — e.g. JJD-01011961-Signature.
✅ Prepare and upload your head-and-shoulders photo (plain background).
